At which hospital does Dr. Kefalas Perform Surgery?


Dr. Kefalas and Cami Kistenfeger  have privileges at St. Mary’s Hospital and Decatur Memorial Hospital

What information will I need to bring to my initial visit?


At the time of your initial appointment, we ask that you bring the following items: Photo ID, insurance cards, medication list, and all pertinent medical records from your family physician. In order for you to be seen in a more timely manner, we ask that you print off and complete all forms and bring them completed with you to your appointment if possible.

Do I need a referral to schedule an appointment?


Dr. Kefalas does not require a referral to schedule an appointment. However, in some cases an insurance company will require a referral by your family physician. Please consult your insurance plan for additional information.

Which Insurance plans do you accept?


Dr. Kefalas is a preferred provider with most insurance plans. If you have any questions regarding our participation with your plan, please call the billing office at (217) 425-2600.

Will I be required to pay my co-pay at the time of service?


Yes, we ask that yo pay your co-pay at the time of service.
